Online survey clarification?

EGO sent me survey to respond to for their customer service, but it does not clarify what a 1 rating or a 5 rating is.  
Is 1 good or bad, or is 5 good or bad on their survey?    

Normally 1 would be bad, but the next question has the first entry being 'very satisfied'.  
There are no instructions on their survey.    
I imagine they are getting bad data based on how they made this survey.

I've never taken the survey, but if it is how you say then some clarification could go a long way!

No instructions at all or definition for the numbers.  What I posted is a copy and paste from it.
